What is the new patient form?

The new patient form is a questionnaire you send to the patient before their first appointment so you can get to know them better and make the most of the appointment time for topics related to the meal plan.

It’s fully configurable: you choose the form’s fields, and you can edit it whenever you want.

The patient must complete it before their first appointment, and as soon as they do, the information they entered will appear in their profile, so you’ll already know a lot about the patient before they arrive for the appointment.
